In conclusion, this work identifies GADD45A as a multimodal regulator of cisplatin ototoxicity through three convergent mechanisms: 1) JNK-mediated apoptotic regulation, 2) autophagic pathway activation, and 3) NF-κB1-dependent inflammatory suppression. The clinical relevance of these findings is underscored by successful in vivo protection in Gadd45a knockout models, suggesting that the targeted inhibition of Gadd45a signaling could preserve auditory function during cisplatin chemotherapy. Future investigations should focus on temporally specific GADD45A expression modulation and advanced delivery systems to translate these findings into clinically viable otoprotective strategies.
