In the evolving field of cancer treatment, proton therapy is a promising modality owing to its high efficacy and low toxicity. This review highlights that proton therapy selectively targets cancer cells, spares healthy tissue, and induces a unique immune response in peripheral blood immune cells, tumor cells, and the TIME. In addition, combining proton therapy with immunotherapy and/or chemotherapy may benefit patients with various types of cancer. We hope that this review will provide a better understanding of the impact of proton therapy on the immune system and shed light on potential combination treatment strategies.
