In summary, we report a case of complex fatal self-harming in the setting of isolated psilocybin ingestion, in an individual who is a regular user of magic mushrooms without a history of prior suicidal ideation or attempts. This underscores the potential harm of psilocybin and the uncertainty of manner of death certification in cases lacking prior demonstrated intent to self-harm.
